Professional Care by Caring Professionals
Transitions Nursing and Rehabilitation Center is a 55-bed skilled nursing facility, certified by Medicare and Medicaid. We have a very high RN average, along with our qualified LPN’s and CNA’s providing care.
“The staff is very good. They treat my wife real
good. It’s a nice home away from home.”
Luther Marquette “This is the best place I’ve been with people that
love me more than any other nursing home I’ve
lived in.” Elaine “There are good Nurses and CNAs here and the
therapy dept is excellent.” Richard “My mother is well taken care of. When my mother
is having a bad day, the CNA’s will sit with her until
I get here.” Sharrae Howard “Out of the three nursing homes I have been in, this
is my favorite. I can kid around with the staff
and laugh.” Jim
